Benefits Enrollment Wizard - Adding/Modifying Dependents
Some benefits, such as Medical, Dental or Life Insurance, may allow you to add dependents to the benefits. This is determined by your employer. If allowed, you will be presented with the Add Dependent Benefits Coverage action on the Benefits Enrollment Wizard pages. The Add Dependent Benefits Coverage pop-up window allows you to see your existing dependents.

If you select the Add Dependents Benefits Coverage button, you will be presented with an Enroll Dependents window.
If the list of dependents contains all the people you wish to enroll as dependent, then select the check box associated with the dependent(s) you wish to enroll for the benefit coverage and select the Enroll button.
Once you enroll a dependent, the particular dependent will be shown under the Enroll Dependent section on the Benefits Enrollment wizard and will not be available under the Add Dependent Benefits Coverage window.
If you wish to add a dependent that is not displayed in the list on this Enroll Dependents page, select the Manage Dependent button and enter the information for the dependent. Refer the instructions under Adding a New Dependent below for further information.
The Manage Dependents button allows you to add a new dependent or modify the details of the existing dependent.
If you wish to add a dependent that is not listed on the Enroll Dependent page, then click on the Manage Dependent button and perform these steps:
Select the Add icon from the top of the page. A new row will be inserted. You can select the Expand icon to add details for new record or updated the dependent details for an existing dependent.
You can enter the necessary information for the dependent, including First Name, Last Name, Date of Birth, Gender, Relationship, and Social Security Number. If you do not know the Social Security Number or one has not been yet been assigned but has been applied for, you can specify in the SSN Applied For field. Select Yes if the Social Security Number has been applied for.
You may also provide the following optional information: Middle Name, Suffix, Wedding Date, Divorce/Separated Date, SSN Applied For (only if you do not know the SSN), Disabled, and Student.
Once you have made all your changes, select Save to save your changes. Now you can add this newly created dependent to benefits by choosing the Add Dependent Benefits Coverage action on the benefits page.
